Please note that just because we are in network with your insurance company, there is no guarantee that your specific plan covers chiropractic care. You should contact your plan before your visit to confirm coverage. You can find their number on the back of your card. We will attempt to contact your insurance to confirm chiropractic coverage when you come in for your first appointment.
We cannot guarantee that the information they provide us about your plan is accurate because they do not make that guarantee with us, nor do they give us accurate benefit information 100% of the time. We cannot guarantee they will pay. If there is a discrepancy between what we collect at the time of service and your patient responsibility on your EOB, we will either bill you or refund you.
You are responsible for payment if your insurance does not pay.
PLEASE check your EOBs provided by your insurance carrier for “Patient Responsibility” after
each visit. This is what you will owe for each visit. The charges they set forth should line up perfectly
with your account ledger here and amount owing to us. We no longer send monthly billing statements.
Payment is due at the time of service. Statements will be provided upon request at any time
